Your First Visit

To expedite your first visit, please fill out the Patient Information before coming in. Once you fill this out and submit, it will automatically be sent into us and start your chart. This will save you time when you come to the office so that you don’t have to fill in the forms at the office. In addition to Patient information, please fill out Medical Release so that we can get any records necessary from your provider, and review our Privacy Practices and sign off. If you are coming for your Prenatal Screening, we encourage you to watch the videos offered online. These will clearly explain the testing you will be offered by Dr. Sachar for both Prenatal Screening and Carrier Screening. At the office, you will be asked to sign a consent depending upon which testing you are interested in.
